Membership Form for the Durham Down Syndrome Association

The Durham Down syndrome Association is always happy to have new members. Anyone can be a member, families with children or other members of the family who have Down sydnrome, friends, schools, organizations and more. The members of the DDSA are diverse and live not only in Durham Ontario. The DDSA is a non-profit organization. Membership dues collected go directly back into maintaining the association by providing awareness, events, other social activities and much more.

Donations are also welcome, and amounts given of $20.00 or more will receive a tax receipt issued by the DDSA Treasurer.

Membership is $20.00 per family/household and is for the calendar year. You can either download and print the following Membeship PDF form and mail in the completed form with your $20.00 membership fee to:
